Answer — that is death itself staring you in the face.  It wants you to inflate more…

Its a ACOM fenestration…  The wire and balloon managed to squeeze through this tiny ACOM fenestration.  The balloon is “waisting” around it…  inflating more ruptures the fenestration.  Given how critical everything here is, hard to imagine how rupture would not be lethal…

A noncompliant balloon — which some think is safer in vasospasm angioplasty — would have proven terrible here
